On Java Edition, to decorate a shield, you need two items: A banner and a plain shield. Banners can be crafted with colored wool with a stick on a crafting table like you would craft a sign. Some banners can only be found in structures and cannot be crafted, such as the Ominous Banner (Dropped by patrol leaders, raid leaders, and found on Outposts.

In mobile edition, you can block the same way you eat (offhand or not). And in console using the “use” buttons when you can’t do anything else, same for pc.

The closest you can get is making a shield (craft: make a Y with wood planks and put an iron ingot in between the y. plank ingot plank. plank plank plank. (empty) plank (empty) you put it in the slot (in your inventory) beside your character on the other side of where you put on armour.
No blocking the sword has been replaced by the shield. Then have to go to an older version. However, if you have the sword in your hand, as far as I know, you can open your inventory and simply pull out the shield, then you will no longer hold it under your nose in battle.
